The deal, which was preliminarily approved by the court in March 2025, covers all United States residents who received notice that their private information may have been impacted in the Imagine360 data breach.
To receive an Imagine360 data breach settlement payout, class members must file a valid claim form by July 31, 2025.
Claim forms can be submitted online on this page, or you may download a PDF claim form to print, complete and return by mail.
Imagine360 class members will need to provide the unique ID printed on the postcard notice they should have received in the mail. If you did not receive a notice or have lost yours, contact the settlement administrator using the phone number listed here.

Class members who submit a valid, timely claim form will be eligible to receive three years of free credit monitoring services.
Consumers can also file a claim to get up to $5,000 in reimbursement for documented losses incurred as a result of the Imagine360 data breach, such as bank fees, long distance phone charges, cell phone charges, postage, gasoline for local travel, and fees for credit reports, credit monitoring or other identity theft insurance products.
In lieu of reimbursement, class members can elect to receive a flat cash payment, estimated to be around $75. The settlement website notes that cash payments may be adjusted on a pro rata basis depending on how many valid claims are submitted.
The court will decide whether to grant final approval to the deal at a hearing on August 15, 2025. Class action settlement benefits are typically issued to eligible class members once a settlement receives final approval from the court and after any appeals are resolved.
Initially filed in July 2023, the Imagine360 lawsuit claimed that unauthorized access to the company’s Citrix file-sharing solution occurred around January 30 of that year due to the defendant’s cybersecurity failures. The settlement website says that the incident resulted in the exposure of individuals’ names, medical information, health insurance information and Social Security numbers.
As part of the settlement, Imagine360, a vendor for employee health plans, has agreed to implement additional data security measures.
